Bästa strategi för trombosprofylax för buk- och bäckenopererade vuxna patienter

Vid större kirurgiska ingrepp ökar risken för blodproppar i benen (djup ventrombos, DVT) och lungorna (lungemboli). För att förhindra uppkomst av blodproppar kan patienten få förebyggande behandling (trombosprofylax) såsom lågmolekylärt heparin (LMH), graderade kompressionsstrumpor (stödstrumpor), kompressionspump och sekventiell pneumatisk kompressionspump.

Sammanfattning av Upplysningstjänstens svar
Upplysningstjänsten har identifierat två systematiska översikter och tre randomiserade kontrollerade studier (RCT). Författarna har undersökt kombinationen av heparin och graderade kompressionsstrumpor jämfört med heparin vid bukkirurgi. Vi fann inga studier som direkt utvärderat kombinationer av LMH och/eller kompressionspump eller sekventiell pneumatisk kompressionspump vid buk- och bäckenoperationer.
I översikterna drar författarna slutsatsen att optimal profylax vid tjock- och ändtarmskirurgi är en kombination av graderade kompressionsstrumpor och heparin och att heparin kan bytas ut mot LMH.
I en RCT drog författarna slutsatsen att det inte var någon signifikant skillnad i förekomsten av DVT efter kombinationsbehandling med heparin och kompressionsstrumpor jämfört mot heparin ensamt. I två andra RCT:er drar författarna slutsatsen att en kombination av heparin och graderade kompressionsstrumpor är effektivare än heparin ensamt för att minska förekomsten av DVT efter operation.
SBU har inte tagit ställning i sakfrågan eftersom de enskilda studiernas kvalitet inte bedömts och resultaten inte vägts samman. Här redovisas därför endast de enskilda författarnas slutsatser.
