Low level laser therapy in tennis elbow

Tennis elbow, or lateral epicondylitis, is a common pain condition found in 1 to 2 percent of the adult Swedish population. Low level laser therapy is sometimes used as a treatment modality in tennis elbow.

SBU Enquiry Service

Consists of structured literature searches to highlight studies that can address questions received by the SBU Enquiry Service from Swedish healthcare or social service providers. We assess the risk of bias in systematic reviews and when needed also quality and transferability of results in health economic studies. Relevant references are compiled by an SBU staff member, in consultation with an external expert when needed.

Question

What systematic reviews are published regarding low level laser therapy for tennis elbow?

Identified literature

Nine relevant systematic reviews were identified. One of them had moderate risk for bias and is presented below [1]. Eight had high risk of bias and is not presented further [2-9].

Table 1. Systematic reviews with low/medium risk of bias
CCT = Controlled clinical trial; CI = Confidence Interval; LLLT = Lowlevel laser therapy; n = Number of studies; WMD = Weighted mean difference
Included studies Population/Intervention Outcome and Results
Tumilty et al. 2010, [1] Low level laser treatment of tendinopathy: a systematic review with meta-analysis
13 CCTs, 536 subjects Population: Epicondylitis

Intervention:
LLLT Wavelength: 632.8 to 1060 nm Power density: 60 to 400 mW/cm2 Energy density: 1.8 to 150 J/cm2

Control:
Placebo (n=8) or other active control (n=5) such as other electrotherapy modalities, ultrasound, and corticosteroid injection.
Meta-analysis:

Grip strength (n = 4): WMD: 9.59 kg (95% CI, 5.90 to 13.27) Average grip strength in intervention groups was between 29.57 and 56.30 kg after treatment.

Average grip strength in control groups was between 21.61 and 43.60 kg after placebo.

Pain scores (n=5):
Data could not be pooled
Authors' conclusion:
“This study found conflicting evidence as to the effectiveness of LLLT in the treatment of tendinopathy.”
“LLLT can potentially be effective in treating tendinopathy when recommended dosages are used.”
“Poor blinding procedures, lack of randomization, and lack of intention to treat analysis may have increased potential biases and weakened the scientific merit of the works reviewed. “
