Discharge planning conference prior to discharge of the elderly

Many patients who are admitted to hospital still require care after discharge. A group that often require several different help efforts after discharge is older people with multiple illnesses. A discharge planning conference where the patient’s needs are assessed and arrangements are made is a way to try to ensure a safe discharge.
